Submit a new provider

It’s free! All providers that recognize themselves in one of the categories are invited to apply online and create a free basic profile of their institutions. The application process consists of two phases. First phase is initiated when the provider’s representative wants to create a profile, clicks on the Submit New Provider button and selects one of the main (basic) categories (hospitals, group practices, laboratories, pharma, dentists, spa). Once the category is selected, the application will ask for basic information such as: name of the provider, address, city and country, type of establishment, name of director/manager, name of contact person and contact details. Applicants should choose their initial username and password, locate the facility on the map, and once finished click on the Submit button. Once submitted you will receive an e-mail from our customer service within 24 hours in order to verify the data submitted within the application forms. After verifying the data, you will be sent a Form that needs to be signed by the director/manager assigning a person in charge to edit and manage your Healthgrouper profile. The Form could be sent by fax or a scanned copy by e-mail or by regular mail. The HG team will review and edit the document, and will send it to the provider representative for a final authorization. Once approved by the provider, the profile will be published online and the provider representative may access it and edit sections where they have administrative access at any time. The main profile sections are: basic information; overview; treatments, collaborations, specialists associated with the provider.
